What insurances do you take?

Dr. Pardo is in-network with most PPO insurance plans, though not all. However, we are happy to accept and see patients with all PPO insurances, including but not limited to:

Delta Premier, Delta PPO, Guardian, Metlife, Aetna, Blue Cross, United Concordia, Humana, GEHA, Cigna, Blue Shield and more.

 

We DO NOT accept Medi-CAL or HMO plans.

What do I need to make an appointment as a new patient?

If you are a new insured patient, we need the following:

  • Patient Name and Subscriber Name (if different)

  • Patients DOB and Subscriber DOB

  • Insurance Carrier and Subscriber ID or ss#

  • Email and telephone

What do I need to bring to my first appointment?

We need a form of photo ID, and the New Patient forms filled out before your appointment.
